Patient Registration via Paper Form

Use this workflow when a patient cannot or will not register via QR code.

1. 1

   Patient attends clinic appointment.

   Patient cannot or does not scan the BnTRx QR code.

3. 3

   Clinic provides the Co-payment Authorisation and Consent paper form (download: https://www.bntrx.com.au/resources/co-payment-authorisation-and-consent-form.pdf).

   Patient completes the form and returns it to clinic admin.

5. 5

   Clinic adds patient details in BnTRx using the Patient Management workflow.
